Referral Form
Montclair to College (MTC) students are eligible to receive a two-year scholarship to Chaffey College if the program requirements are fulfilled yearly, as outlined, prior to graduation from Montclair High School (MHS). The MTC program is brought to MHS Students in collaboration with the Montclair Community Foundation, City of Montclair, Chaffey College, and Chaffey Joint Union High School District. 

  • Complete the MTC online application by 3/15/19
  • Complete 5 hours of community service and submit the tracking log by 4/1/19
  • Attend MTC workshops and activities
  • Remain enrolled at MHS, and be aware of the cumulative 2.0 GPA requirement and have no significant disciplinary action upon graduation

The Association Educational Foundation, through the generosity of its donors, provides merit-based scholarships for undergraduate students pursuing degrees related to the industry. Scholarships awards range from $2,500 to $10,000.

Beau Iske Insurance Agency Inc has been serving the Millard area for insurance since 1962. Representing State Farm Insurance Companies, we are proud of our history and roots here in the Millard community. Gerald Iske started with insurance in 1962 at 128th and Q Street. In 1990 Beau Iske (his son) started his agency at 138th and Q Street. Both Father and Son have been very active in the Millard community and in appreciation of their longevity and success, would like to give back with this scholarship to help out the next potential business student. Our goal is to find a student in good academic standing, active in activities in school and working in accordance with teachers and administration. Our goal is to help them achieve their dreams with this money used toward their tuition.

The Credit Union Scholarship application for the academic year is available:

The Credit Union will award six (6) $1500 scholarships to members and three (3) to employees’ family members or co-op employees who are eligible high school seniors.

Eligible applicants must be:
  • a member of CU/AECU/PCU (account must be in applicant's name)
  • a graduating high school senior, and
  • accepted as an incoming freshman at an accredited college or university

The deadline (or postmark date) for scholarship entries is March 31, 2019.
Winning members will be notified no later than May 15th 2019.

Dr. Jon Lopez was a dedicated educator who believed that the best leaders were caring leaders. He was an extraordinary advocate for his students, staff and community. He worked in public education as a teacher in Millard Public Schools, and was Principal at Millard South. He went on to become a Superintendent of Schools in Beatrice but always had a special place in his heart for the students of Millard. Dr. Lopez was devoted to getting to know all who were involved with Millard South. He was a man who believed all students are the makers of their destiny despite past hardships. He would be proud to be part of helping students pursue their dreams of making a difference through education. He was always appreciative of the people who helped him in his journey to become a successful leader. Dr. Lopez passed away after a valiant and brave battle with pancreatic cancer. He viewed any of life’s challenges as opportunities to grow as a person. This scholarship was founded with the intent of honoring his memory by supporting students who are willing to do what is takes to shape their destiny.

The scholarship must be used for undergraduate study at an accredited university, college, or other post-secondary educational institution of the recipient’s choice. The winner must begin attendance at the selected institution by October in the year of high school graduation.  Two scholarships with a value of $2,500.00 each will be awarded to one female and one male senior student athlete.

“Serving the Children of the World” is the goal of this international service organization of volunteers. The Kiwanis Club of Southwest Omaha is one of 13,000 local Kiwanis clubs throughout North America and in more than 80 countries. Kiwanis members strive to identify issues and community needs, with a special emphasis on children, and respond with projects designed to promote the welfare of children and society as a whole. Our club’s membership includes men and women from a variety of businesses and professions, as well as retired, who pledge to uphold the ethical and humanitarian goals of Kiwanis and who have a desire and the ability to serve. We volunteer for community service that helps others, such as serving meals at the Open Door Mission, delivering food baskets at Thanksgiving, helping with Special Olympics, and reading with students in elementary reading programs. Kiwanis also sponsors youth community service clubs, “K-Kids” at Rockwell elementary, “Builders Clubs” at Andersen middle school, “Key Club” at Millard West high school, “Circle K” at UNO, all of which teach community service and leadership skills to young people and adults. Recipients of the Northern New York, Herring, and Visiting Nurse scholarships are chosen based on student’s combined regents scores, cumulative high school GPA, rank in class, personal data, essay scores and applicant appraisals. The mentioned criteria are entered into a database to produce a numerical ranking of all freshman applicants. Remaining Foundation restricted scholarships are awarded based on donor driven criteria.

The Foundation was established to provide scholarships toward tuition for qualified high school graduates at selected high schools in Northwest Indiana. Scholarships are awarded to high school seniors to recognize academic achievement, demonstrated leadership, and future potential.

Of special interest to the Foundation is the economic development and general welfare of Northwest Indiana. The Foundation believes a better educated population will enhance the quality of life for all its citizens. While not a specific criteria for receiving a scholarship, some of the ideals of the Foundation will be realized if those who benefited from the scholarship program live and work in Northwest Indiana following graduation from college.

About the Lilly Endowment Community Scholarship:

The purpose of the Lilly Endowment Community Scholarship Program is to help raise the level of educational attainment in Indiana and to increase the ability of Indiana community foundations to enhance the quality of life of the state’s residents.

You are invited to apply for the Reaching Your Potential scholarship because you were referred by a community agency or school – and because you have demonstrated the potential to succeed.

Reaching Your Potential scholarship recipients attending a two-year college will receive $2,700, and recipients attending a four-year college will receive $5,500. The scholarships are renewable up to a Bachelor’s degree based on satisfactory academic performance.
